Regulation 7.4.4: All staff shall complete fifteen (15) clock hours of annual professional development activities as required in the rule 7.4.2 of these regulations. Substitutes filling a staff position for less than thirty (30) consecutive days within a 365 day period shall be exempt.
“The program did not provide documentation of the following annual professional development that was not found in BFIS. Staff A appears to be missing fifteen (15) hours between 6/12/2024 and 6/11/2025, Staff B appears to be missing twelve (12) hours between 8/29/2024 and 8/28/2025, Staff C seems to be missing three (3) hours between 8/23/2024 and 8/22/2025. Regulation 6.2.6.1: Staff shall be able to hear all children of pre-kindergarten age and younger at all times, shall be able to see the children with a quick glance, and shall be able to physically respond immediately.
“On 8/7/2025, Child X left the group unnoticed for approximately two (2) minutes and made their way to the sidewalk on North Avenue. Once Child X was noticed missing, it took approximately two (2) additional minutes before staff had visual sight of them, talking to a friend of the family on the sidewalk. This is a serious violation. Corrective Action: After the incident, the program contacted the Division as required. In addition, the director completed the active supervision training provided to Head Start programs and completed the same training with staff. The program has added a bright orange climbing rope as a visual barrier along the back and one side of the base camp, and has been teaching children not to go beyond that rope without a teacher. The secondary path that Child X took has been made an emergency only path and is no longer used on a regular basis. The program has added walkie talkies for communication, and has designated one staff person at all times to have eyes on the entire group, continually counting children. Please complete the following additional corrective action. 1. Send this licensing visit report (either a hard copy or email a digital copy) to all families that were enrolled at the time of the incident. 2. Email Licensor [name] a list of all children enrolled at the time of the incident, including parent/guardian names for all children and their phone numbers. 3. Modify the missing child protocol to include a 30-second sweep before contacting emergency services. 4. Send copies of the new supervision protocol and missing child protocol to Licensor [name].”

“At the time of the visit, three (3) staff who had been employed for over sixty (60) days had not completed all portions of their background clearances. Staff B had not yet sent out of state paperwork to the Division. Staff C had not yet sent out of state paperwork to the Division and it is unclear whether they had submitted to fingerprinting.
