Regulation 5.2.3.11: Each diaper changing procedure shall include: * Preparation: assemble supplies, staff member washes hands, prepares changing surface with either a disposable covering or ensure that the surface was recently cleaned and disinfected and bring the child to the diaper changing area. * Dirty Phase: remove soiled diaper and cleanse diaper area of the child. * Clean Phase: put on clean diaper, dress child, and wash the child's hands. * Clean up: clean and disinfect the diaper changing surface, dispose of diaper, and staff member washes their own hands. * Record diaper change.

“This violation was cited at the 6/13/2025 compliance visit because the provider did not wash her own hands before a diaper change and did not wash the child's hands after the same diaper change. The corrective action was due 7/16/2025. This was also cited as a talking point at the compliance visit on 6/26/2024. Corrective action was not received from the provider. Follow up emails were sent on 7/24/2025 and 1/27/2026. At the licensing visit on 2/9/2026, the provider stated that she just forgot to wash children's hands after a diaper change during one visit, and that she washes children's hands after diaper changes consistently. The provider stated that she cannot leave the children alone to wash her own hands before the diaper change and so she uses gloves. This is a repeat violation. Corrective Action: Please email Licensor [name] a written plan that demonstrates your understanding of the intent of this rule and the system you will implement to ensure that this rule is consistently followed moving forward. Failure to submit corrective action on time may result in further regulatory action which may impact your license.”

Regulation 7.4.4: All staff, with the exception of substitutes, shall complete fifteen (15) clock hours of annual professional development activities as required in the rule 7.4.2 of these regulations.

“A review of the provider's BFIS quality and credentialing account, along with email correspondence from July 2024 indicates that the provider completed seven (7) hours of annual professional development between 4/14/2024 and 4/13/2025 and eleven (11) hours between 4/14/2023 and 4/13/2024 for a total of eighteen (18) hours out of the required thirty (30) hours for the two-year period. This was cited as a violation at the 6/26/2024 visit as the provider had not completed the full fifteen (15) hours of annual professional development for the 4/14/2023 - 4/13/2024 period. The provider did not submit the required corrective action to Licensor [name] for the 2024 violation, which was due on 7/26/2024 or the 2025 violation, which was due on 7/16/2025. Follow up emails were sent on 7/24/2025 and 1/27/2026, and corrective action was still not received. During the licensing visit on 2/9/2026, the provider provided documentation of two (2) training courses completed on 8/26/2025 and 1/22/2026 totaling two (2) hours. The provider said she would look through her paperwork for training certificates during naptime and would send them to Licensor [name]. No certificates were received. This is a repeat violation. Corrective Action: Please send the two (2) certificates for the training already completed to Northern Lights at CCV via Direct Send using the information provided at the 2/9/2026 visit. Please complete ten (10) hours of professional development by 2/25/2026 and submit the certificates to Northern Lights at CCV using Direct Send. By 2/25/2026, email a plan to Licensor [name] describing how you will complete the remaining fifteen (15) hours by 4/13/2026 and submit the certificates to Northern Lights at CCV using Direct Send. In the plan, include the title and number of hours for each training course and what date the training will be completed. You can contact Northern Lights at CCV Resource Advisor Adri Taylo-Behrman at adri.taylo-behrman@ccv.edu or visit the Northern Lights at CCV Training Types and Criteria page at https://northernlightsccv.org/training/training-types-and-criteria/ to find qualifying training opportunities. “A violation was cited at the 6/13/2025 compliance visit because several documents have not been sent to Northern Lights at CCV. Corrective action due on 7/16/2025 was not received from the provider to show that documents were sent to Northern Lights at CCV. Follow up emails were sent on 7/24/2025 and 1/27/2026.
