“A Program Staff submitted a record check authorization form and was sent a letter on 11/7/2024 with the Fingerprint Authorization form and letter stating how to complete the out of state record check. The results of the out of state check has not been completed. Corrective Action: Have the identified Program Staff complete a new record check aothorization form and then complete the out of state check they are sent as required.”
