“On 05/19/2026 Child A did not arrive at the program as expected. The Director stated that when they noticed Child A was not present they went to the school's office and learned that a substitute teacher mistakenly sent Child A home on the school bus. The bus company was called and confirmed that Child A was dropped off at home. The Director stated after learning what happened they became sidetracked and did not contact Child A's parents or emergency contacts. Corrective Action: The program will create a written procedure that clearly outlines the steps that will be immediately taken once staff identify that a child did not arrive at the program as expected. It will also identify next steps if the parent or emergency contacts do not answer their phones. Submit a copy of this procedure to Licensor [name] for review. Once approved this procedure will be reviewed with all current staff and with new staff when they are hired. Status: Appeal Period.”
