Regulation 3.4.7.3: Within six (6) months of the initial date of employment; staff and auxiliary staff left alone with children and/or counted in staff/child ratio as specified in the rule 6.2.1.8 of these regulations, and the program director shall maintain an up-to-date BFIS Quality and Credential Account. Documentation, verification of qualifications, and all annual professional development activities as specified in the rules in sections 7.3 and 7.4 of these regulations shall be submitted to Northern Lights at CCV to be verified and maintained in BFIS.

“A review of staff BFIS accounts, showed that accounts are not being kept up to date as required. Corrective Action: Submit a plan on how all missing documents will be sent to Northern Lights at CCV Direct Send and when they will be submitted. Email confirmation of the plan and how staff will be supervised to ensure that BFIS accounts are maintained.”

Regulation 5.2.3.11: Each diaper changing procedure shall include: * Preparation: assemble supplies, staff member washes hands, prepares changing surface with either a disposable covering or ensure that the surface was recently cleaned and disinfected and bring the child to the diaper changing area. * Dirty Phase: remove soiled diaper, dispose of diaper, and cleanse diaper area of the child. * Clean Phase: put on clean diaper, dress child, and wash the child's hands. * Clean up: clean and disinfect the diaper changing surface, and staff member washes their own hands. * Record diaper change.

“Two diaper changes were observed in an infant classroom. During this time staff did not wash hands prior to changing diapers and did not wash the hands of the infants after they had their diapers changed. Corrective Action: Immediately instruct staff to always wash hands prior to changing diapers, before putting on gloves and to wash the hands of infants after having their diapers changed. Submit an email on how staff have been retrained on the diaper changing procedure and that they are washing hands when required. Include how staff will be supervised to ensure handwashing is occurring when necessary.”

Regulation 7.2.4: The licensee shall ensure that no person shall be left alone with children without approval from the Division which shall be based on the results of the background check to include fingerprinting.

“There were two (2) staff present who have not received the results of the fingerprint supported clearance and are being left alone with children. No staff member may be left alone with children prior to the Child Development Division receiving the results of a fingerprint supported background check. Corrective Action: Immediately adjust the programs practices and/or schedule so that staff are not left alone with children until the results of the fingerprint supported clearance is received. Email this licensor with the steps that will be taken to ensure that staff without fingerprint results will not be left alone with children.”

Regulation 5.4.1.8: Bedding used by a child shall be stored in a manner that prevents contact with bedding used by other children.

“The bedding used for children is stored together inside storage bins in the two and three year old classrooms. Corrective Action: Effective immediately, store children's bedding in individual bags or store in a way that prevents the bedding from touching as discussed.”
