      “Staff A and Staff B were each working alone with children without approval from the Division. The Fingerprinting Authorization Form for staff A was issued on 9/11/2019 and the one for staff B on 10/1/2019. Corrective Action: Staff A and Staff B shall submit to fingerprinting no later than 4/06/2019 and shall not be left alone without approval from the Division based on results of fingerprinting.”
