Regulation 5.11: The licensee shall ensure that any new individuals regularly present at the Afterschool Child Care Program submit a Records Check Authorization form to the Division prior to working with children in the Afterschool Child Care Program.

“Staff A was present at the program and the program had not submitted a record check authorization form as of the date of the visit. Corrective Action: The program submitted the record check authorization form for Staff A after the visit. Once the fingerprint authorization certificate has been issued, ensure that Staff A completes fingerprints and, if applicable, out of state child abuse and neglect registry checks for any states they have lived in during the past five (5) years. By 9/19/2025, email Licensor [name] Staff A's fingerprint receipt or confirm that an appointment has been made for fingerprinting. In addition, if out of state child abuse and neglect registry checks are necessary, send confirmation that Staff A has followed through with requirements for the check. Until the Division sends final clearance, Staff A must not be left alone with children. Moving forward, ensure that all new or returning staff submit a record check authorization form to the Division on or before their first day of employment.”
