    • Supervision and ratios

      8VAC20-780-340-A

“REPEAT VIOLATION When staff are supervising children, they shall always ensure their care, protection, and guidance. 1. The Preschool classroom was observed for approximately an hour, during which children were running around the classroom, throwing toys, climbing on the windowsill wedging themselves between the window and diaper table, standing on the arm of a small couch, standing on a bookcase, crying, and attempting to leave the classroom. Child #1 (age 3) pushed Child #2 (age 3) hard into the wall, making an audible thud when Child #2 hit his head against the wall. Child #2 began crying in front of Staff #1. Staff #1 did not respond to the incident and began to walk in another direction away from the children. A child put another child in a headlock. Staff #1 was sitting on the floor behind a bookcase and unable to see the children and Staff #2 was in the bathroom with children. Neither staff were able to intervene nor witness the incident. The child put in the headlock cried out loud while in the headlock. A child hit another in the face with a wooden block, causing that child to cry. Staff #1 was unable to witness the incident because the staff was sitting on the floor by a bookcase, which blocks sight of the children on the other side of it. Child #2 (age 3) chased child #3 (age 3) across the room and then pushed child #3 into the cubbies, causing child #3 to hit his head and cry. 2. Child #4 (age 2) had been receiving services at the center from an outside agency for several months on a weekly basis for approximately an hour each session. The center indicated that on occasion the individual from the outside agency is alone with child #3 for therapy. Administration did not have documentation of the individual’s name or agency or written parent authorization but allowed that individual into the center, the classroom and alone with child #3.”

Administration reported there was a plan in place to deter biting by assigning specific children identified as biters to certain staff to ?break up the biters,? referred to as pocket buddies. 3. Although administration reported that four identified biters in the Toddler classroom had been assigned to specific staff as pocket buddies, three observations of the classroom (one on May 14, 2024 and two on June 20, 2024) revealed that the pocket buddy system is not being implemented. On May 14, 2024, an identified biter was observed climbing into close quarters of an open cabinet with other children without supervision or intervention from staff.”
