Regulation 3.3.4.2: Each child's file shall be complete with all required information within the first week that the child begins to attend care. The required annual physical shall be obtained within forty-five (45) days of enrollment as required in rule 5.1.2 of these regulations.
“The children's files were audited, and the required immunization record was not collected for the children at the time of enrollment. The provider stated she didn't have immunization records for enrolled children. The children's file reviewed have been enrolled longer than a week. Corrective Action: By 9/11/2026, collect all children's immunization records and maintain compliance by collecting immunization records as part of the paperwork required for enrollment. Submit a confirmation to this Licensor when all immunization records have been collected. Status: Appeal Period.”
