Regulation 2.3.9.3: The licensee shall ensure that all staff have met ongoing background check requirements by submitting at least once every five (5) years a Records Check Authorization form to the Division and as required shall submit to fingerprinting.

“Staff C and D did not follow through with fingerprinting before the five-year expiration date. For staff D, the due date for fingerprints was misstated in the communication from the Division, and the program is not at fault for the non-compliance. Staff E was fingerprinted on 8/4/2025 but did not send a record check authorization form to the Division until 10/13/2025, which was after the 9/21/2025 expiration date of their fingerprints. Corrective Action: Since the visit, a record check authorization form has been sent to the Division for Staff C, and the program director stated that Staff C has a fingerprinting appointment set for the week of 5/25/2026. The director stated that Staff D has sent a record check authorization form to the Division and will make an appointment to be fingerprinted when the fingerprint authorization certificate has been issued. Email Licensor [name] to confirm that Staff C and D have been fingerprinted, and provide the date, location, and TVT number for both sets of fingerprints. For Staff E, contact [name] at Jacqueline.R.Davis@vermont.gov with the following information for Staff B: full name, date of birth, date fingerprinted, location fingerprinted, and the TVT number.
