“There were two (2) paraprofessionals in one of the classrooms who do not appear on the program's associated parties list. Corrective Action: By 4/19/2024, ensure that the two (2) identified staff complete and send a record check authorization form to the Division and comply with fingerprinting and out of state child abuse and neglect registry checks if applicable. Moving forward, ensure that all staff working in the room with children submit a record check authorization form to the Division and follow through with the appropriate level of clearance based on their position in the classroom.”
