“Upon review of the associated parties list, it was determined that there was a total of three (3) individuals listed on the associated parties list that are not affiliated with the program. It is required to keep the associated parties list up to date within five (5) business days of any changes. Corrective Action: By 1/31/2020, update the program's associated parties list by removing any individuals that are no longer associated with the program. In the future, ensure that the associated parties list is updated within 5 business days of anyone leaving or coming to the program.”

Corrective Action: The program will develop a system to ensure the arrival time and departure time of all children will be accurately documented using a time in and time out system.”
