    • Staff training and qualifications

Regulation 3.4.7.3: Within six (6) months of the initial date of employment; staff and auxiliary staff left alone with children and/or counted in staff/child ratio as specified in the rule 6.2.1.8 of these regulations, and the program director shall maintain an up-to-date BFIS Quality and Credential Account. Documentation, verification of qualifications, and all annual professional development activities as specified in the rules in sections 7.3 and 7.4 of these regulations shall be submitted to Northern Lights at CCV to be verified and maintained in BFIS.

“An audit of staff BFIS quality and credential accounts found that they are not up to date with all required documentation. Corrective Action: Since this licensing visit, the program has submitted much of the missing documentation to be entered in BFIS quality and credential accounts. The director is planning on creating a new system for staff to maintain their own BFIS accounts. Submit confirmation of this new plan and how staff have been trained to this licensor by 7/8/2024”

    • Safe sleep

Regulation 5.4.1.5: Children shall be supervised by staff while napping or resting. Staff shall directly observe a sleeping infant at least every fifteen (15) minutes to check the infant's face, view the color of the infant's skin, and check on the infant's breathing.

“It was observed that an infant was sleeping in a nap room from 10:45am-11:05 am without a 15 minute in person check occurring as required. When the staff person was asked about checking on sleeping infants, they said that they could hear when the infants woke up. This is a violation because it is required for all infants to be checked in person every 15 minutes while they are sleeping. Corrective Action: Effective immediately, infants are to be directly observed every 15 minutes while they are sleeping. Ensure all staff who are assigned to work in the infant room, are trained in safe sleep practices including 15 minute in person checks while infants are sleeping.”
