“The center is required to maintain a written record of daily attendance for each group of children that documents the arrival and departure of each child in care as it occurs. At approximately 7:18am on 4/16/26 12 children were observed in care. The written attendance record contained documentation of 11 children’s arrival times and a departure time of 7:50am for the same 11 children. After the Licensing Inspector (LI) pointed out that 12 children were in care, a staff member updated the attendance to include two more children who had arrived previously but had not been signed in on the attendance. The LI pointed out again that 12 children were present, not 13, and the staff member signed out a child who had gone to a program in the school at approximately 7:10am.”
