“Based on interviews with staff, the staff did not ensure that when staff were supervising children, they always ensure their care, protection, and guidance. Evidence At approximately 10:30am on March 21, 2023, the Preschool 2 classroom was outside on the large playground in the back right side of the Center. Staff #3 was on the small playground on the left side in the back of the Center, preparing the infants to go back inside. Three-year-old child #1 ran down the walkway from the large playground, past the small playground and staff #3, and around the corner of the Center. Staff #3 yelled to staff #1 and #2. There was no response from either staff member. Staff #3 called the Center Director from her cell phone. Child #1 entered the area where the trash bins are kept through a broken open gate and exited through the open gate on the other side of the trash bin area, running across the parking lot and across Upper Meadow Drive, approximately 130 feet. The posted speed limit for the road is 25 mph. Center Director immediately ran to the front of the building and brought child #1 back to the playground.
