“A review of staff BFIS credential accounts reflected that 11 out of 13 staff had not yet submitted all required documents to Nothern Lights at CCV as required by this rule. All 11 staff members have been working with the program for at least 180 days. While the director stated that documentation had been previously submitted, she was not able to provide email evidence as such. An email detailing the outstanding staff documents was sent to the director on 05/18/2024. As of 05/31/2024, Licensor [name] had not received documentation of previous submission. As a result, compliance with several other rules was not able to be assesed. These rules include but are not limited to: 7.1.3, 7.1.2.1, 7.1.2.2, and 6.2.1.4. Corrective Action: Immediately plan to submit cover sheets and all required documents for all staff identified in the email sent to the program on 05/18/2024 to Nothern Lights at CCV and copy Licensor [name] on those email submissions. If you need assistance with this process please reach out to Resource Advisor Ammie Collins. Her email is ammie.collins@ccv.edu.
