OCCL Standards Complaint (Substantiated)
“The Office of Child Care Licensing (OCCL) received a complaint on May 7, 2026, regarding concerns of inappropriate behavior supports. Prior to the complaint, the facility submitted a self-report to Licensing Specialist (LS) #1 regarding the incident and provided documentation related to Staff #1 s termination and video footage for review. On May 14, 2026, LS #1 reviewed the video footage as part of the investigation. During the review, Staff #1 was observed physically handling Child #1 by pulling Child #1 by the arm, lifting Child #1 by both arms, and forcefully placing Child #1 into a seated position against the wall. Based on video footage reviewed during the investigation, this complaint is substantiated for not using positive behavior management practices. The employment of Staff #1 was terminated by the facility as a result of this incident.”
OCCL Standards Complaint (Substantiated)
“The Office of Child Care Licensing (OCCL) received a complaint on April 22, 2026, regarding concerns of improper diapering practices, staff not using positive behavior management practices, staff cell phone usage, handling of parent complaints, and lack of supervision. On April 30, 2026, Licensing Specialist (LS) #1 conducted an unannounced investigation at the facility. During the investigation, LS #1 interviewed Administrator #1, Administrator #2, Staff #1, and Staff #2 regarding classroom routines, diapering practices, supervision, classroom management, and cell phone usage. LS #1 also reviewed video footage from April 21, 2026, related to Child #1 s classroom transitions and pickup from the facility. Based on interviews conducted and video footage reviewed during the investigation, this complaint is substantiated for improper diapering practices because video footage showed Child #1 did not receive a diaper change prior to leaving the facility on April 21, 2026. This complaint is substantiated for concerns regarding positive behavior management because multiple staff members and administration acknowledged prior concerns and coaching related to Staff #1 s classroom management, tone, and volume within the classroom. This complaint is substantiated for staff cell phone usage while caring for children because administration and Staff #2 acknowledged staff members use cell phones within the classroom. This complaint is substantiated for lack of supervision because administration stated individual children are occasionally permitted to use the restroom that is not within the classroom while staff supervise from the doorway. There is no evidence to substantiate this complaint for not following policies for handling parent complaints based on interviews and review of video footage. Parent #1 was seen walking past administration without speaking to them on the date of this incident, and administration interviewed stated they were not made aware of any incidents until the end of the day.”
OCCL Standards Complaint (Substantiated)
“The Office of Child Care Licensing (OCCL) received a complaint on March 11, 2026, regarding ratios and inconsistent support from administration to maintain ratio compliance. An investigation was conducted on March 16, 2026. Based on an interview and a review of video footage, this complaint is substantiated for incorrect staff-to-child ratios. Although children remained within sight of Staff #1, the staff left the immediate outside play area two times within a ten-minute period, leaving one staff alone with nine two-year-old children. When Administrator #1 observed what was happening, they intervened and corrected the situation. The center terminated employment for Staff #1.”

OCCL Standards Complaint (Substantiated)
“The Office of Child Care Licensing (OCCL) received a complaint on November 14, 2025, regarding concerns involving safe sleep practices in the infant classroom, improper discipline, failure to provide an injury report, and not following termination policies. An investigation was conducted on November 17, 2025, during which Licensing Specialists (LS) #1 and #2 interviewed Administrator #1, Administrator #2, and Staff #1, obtained documentation, and reviewed video footage. Based on the information obtained, there is no evidence to substantiate the allegations of improper discipline and lack of an incident report from said improper discipline. LS #1 and LS #2 reviewed the footage of this incident, and Administrator #1 and Administrator #2 explained there was not a scratch on the child s chest. No forceful or inappropriate physical contact was viewed between Staff #2 and Child #2. This complaint is substantiated for not following safe sleep practices, as Staff #1 told LS #1, and Administrator #1 confirmed safe sleep checks were not being completed or documented during infant nap times. Additionally, Staff #1 noted children some children need a full five minutes to transition to a crib after falling asleep in other equipment, rather than transferring them immediately. This complaint is substantiated for a lack of behavioral supports for Child #2, as Administrator #1 stated documentation had been collected on Child #2 s behaviors, but no supports or plans for supports were in place, and noted staff were overwhelmed with the behavior and did not know how to address them. There is no evidence to substantiate this complaint for not following termination policies based on review of the letter provided to Parent #1 regarding termination of care for their children, as well as review of the facility s termination policies in their parent handbook. The parent handbook states immediate termination may occur due to bullying, inappropriate behavior, or disrespect towards children and employees, which are concerns noted in the termination letter provided to Parent #1. An additional concern noted during this investigation was Staff #1 was alone in the infant classroom without the required DEEDS Early Learning qualification. An additional concern noted was the length of time infants spent in confining equipment, as an infant was observed in confining equipment for an hour and a half on November 6, 2025. Two additional concerns noted were there was no lesson plan posted in the infant classroom and lighting in two classrooms was insufficient during nap time, as it was too dark to view all individual children.”
OCCL Standards Complaint (Substantiated)
“The Office of Child Care Licensing (OCCL) received a complaint on November 20, 2025, regarding concerns of unqualified staff working with infants, evacuation drills not being conducted, lesson plans not being posted in the infant classroom, infant feeding schedules not being updated, the posted menu not being followed, and staff using cell phones while responsible for caring for children. An investigation was conducted on December 2, 2025, during which Licensing Specialists (LS) #1 and #2 interviewed Administrator #1 and Staff #1, obtained documentation, observed the infant classroom, and reviewed the food menu. This complaint is substantiated for lack of qualified staff in the infant classroom because there were two staff qualified as Early Childhood (EC) Interns through DEEDS Early Learning present in the infant classroom at the time of the investigation, and the third helping staff member is also an EC Intern. This complaint is substantiated for failure to document evacuation drills, as Administrator #1 stated a November evacuation drill was conducted but no documentation was available. This complaint is substantiated for lesson plans not being properly posted in the infant classroom and for two infants missing updated feeding schedules. There is no evidence to substantiate the allegation of not following the menu, as meals are provided by a catering company and minor changes are communicated and documented on the menu. The meal served on the day of this investigation matched the posted menu. There is no evidence to substantiate the allegation of staff using cell phones while caring for children, as no cell phone use was observed during the investigation. Administrator #1 stated staff only use personal cell phones to communicate needs for a bathroom break or emergencies if the classroom tablets are not working. An additional concern noted during this investigation was two classrooms had insufficient lighting during naptime to observe all children.”
OCCL Standards Complaint (Substantiated)
“The Office of Child Care Licensing (OCCL) received a complaint on November 5, 2025, regarding concerns of a lack of supervision, children not being removed from soiled clothing, and not following the center s policy on photographing/videotaping. An investigation was conducted on November 17, 2025. Licensing Specialists (LS) #1 and #2 interviewed Administrator #1 regarding these allegations. Based on the information obtained, the allegation of a lack of supervision is substantiated. Administrator #1 recalled assisting with Child #1 before sunscreen was ingested and stated Staff #1 was overwhelmed and did not intervene appropriately. This complaint is also substantiated for a child not being changed after soiling their clothing. Administrator #1 stated there were no extra clothes available in the center and noted they had contacted Parent #1 several times to bring clothing in for Child #1. This complaint is also substantiated for not following the facility s parent handbook policies on photographing/videotaping due to Staff #1 videotaping Child #1 while exhibiting challenging behavior and posting it on their social media. Upon being made aware of the posting, Administrator #1 stated they had Staff #1 remove the post immediately.”

OCCL Standards Complaint (Substantiated)
“On October 8, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns of improper discipline. This complaint is substantiated because Staff #1 was reprimanded by the facility for using an inappropriate tone toward a child on October 8, 2024, and video footage, though it did not have audio, shows Staff #1 pointing in Child #1 s face while speaking to them. Staff #1 s written reprimand form and a copy of the camera footage were collected at the visit. When asked, Staff #1 stated when a child is exhibiting challenging behaviors, they ask them to sit down once or twice before physically re-directing them by grabbing their hands, which is not best practice. It is concerning that Administrator #1, Staff #1, and Staff #2 were not truthful with OCCL during the investigation. Staff #2 stated she did not see Staff #1 point at Child #1 or hear her yelling, but in the obtained video footage, Staff #1 is standing next to Staff #2 during the incident, which means it would be unlikely for Staff #2 not to see or hear the interaction between Staff #1 and Child #1. Administrator #1 stated during her self-report on October 8, 2024, that Staff #1 did not yell or point her finger in Child #1 s face like Parent #1 said she did , and that Staff #1 was removed from the class for the day. However, Staff #1 was reprimanded on the same day for inappropriate tone towards a child, and both Staff #1 and Administrator #1 signed the Employee Reprimand form.”
OCCL Standards Complaint (Substantiated)
“On July 18, 2024, the Office of Child Care Licensing received a complaint regarding improper discipline and lack of supervision. An investigation was conducted on July 29, 2024. This complaint is substantiated for lack of supervision because the specialists observed video footage of school-age children using the bathroom, which is not directly connected to the classroom, unsupervised. There is no evidence to substantiate this complaint for improper discipline. Administrator #1, Staff #1 and #2 stated Owner #1 does not yell at the children or make them put their heads down and is respected by the children. An additional concern noted during this investigation was the staff using personal cell phones while responsible for caring for children. Three staff in the school-age classroom were observed looking at their cellphones while the children were playing and going to the bathroom.”
OCCL Standards Complaint (Substantiated)
“On February 15, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding improper discipline. An investigation began on February 19, 2024, and was concluded on February 27, 2024. This complaint is substantiated for improper discipline based on the review of camera footage. In the camera footage from February 15, 2024, Staff #2 was observed roughly handling Child #1 in the stairwell. Regulation #20 Ensure all staff are trained in positive behavior supports. Camera footage showed Staff #2 roughly handling a child in the stairwell during the transition from outside play to the classroom. Staff #2 must complete the Texas A &M AgriLife online training "Positive Guidance and Discipline" with center administration sending the certificate to OCCL by April 10, 2024.”

OCCL Standards Complaint (Substantiated)
“On June 14, 2023, the Office of Child Care Licensing (OCCL) received complaints regarding staff administering melatonin, improper discipline, improper release of a child, and lack of supervision. An investigation was conducted beginning June 22, 2023, and ending June 27, 2023. This complaint is substantiated for the improper release of a child and lack of supervision based on interviews conducted and documentation received. A staff member released Child #1 to Adult #1 who was not on the emergency card pick-up list. On June 9, 2023, Child #1 walked alone from the bus stop to the facility due to a staff member not yet being outside. There is no evidence to substantiate staff administering melatonin to children or improper discipline based on interviews and observation. All classrooms and cabinets were checked, and no melatonin was present. All staff denied administering melatonin and stated they never witnessed improper discipline in the center.”
22U Breast Feeding
“Ensure the parent handbook has a procedure for the release of children that includes a policy allowing school-age children to walk home or from the bus stop to the center with written parent permission. Child #1 walked alone from the bus stop to the center without written permission, and Staff #1 admitted to not having a policy in the handbook regarding that procedure. Submit updated parent handbook with required items to OCCL. Ensure the proper release of a child is upheld. Child #1 was released to a guardian who is not on the pick-up list.
