“On March 20, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns of indoor space hazards due to a bookshelf falling on children. An investigation was conducted on March 28, 2025. There is no evidence to substantiate this complaint for indoor space hazards based on interviews conducted and video footage observed. Video footage observed showed Child #1 climbing on the shelf, causing the shelf to fall on both Child #1 and Child #2. The shelf is designed for a preschool environment and doesn t present as being a tipping hazard without the use of physical force. A concern noted at the visit was improper staff-to-child ratio in the preschool classroom upon arrival due to there being 19 children, including two-year-olds, present with two staff members. Additional concerns noted at the visit were: maximum group size for the preschool classroom was exceeded by three children; two staff members were observed in video footage alone with children with no DEEDS Early Learning qualification; and no injury/incident report was completed for Child #1 and Child #2.”
