“Four (4) random children's files were reviewed. Each of the four (4) files were missing at least one item and one folder contained no information on the child or other required documents. Some of the items missing were start dates, general permissions, health information and the child's weekly schedule of attendance. Corrective Action: Before the corrective action due date please gather the missing information and documents discussed during the visit. Only four random files were reviewed, please review all of the children's files and ensure they are complete. Please email this licensor once all of the files are complete. Please create a system that will ensure files are complete and parents are providing required documentation and permissions.”

Regulation 3.3.6.1: The FCCP shall maintain a record of the date of each evacuation drill as required in the rules in section 3.6.2 of these regulations that includes the number of children, staff evacuated, time taken to evacuate, and notation of the drill conducted while children were sleeping or resting. Evacuation records shall be kept for two (2) years.

“While the provider stated that montly evacuation drills are conducted and recorded regularly, the provider was not able to produce the written record attesting to the completion of these drills during the visit. This is a repeat violation. Corrective Action: Immediately plan to record monthly evacuation drills and keep that written record in a spot that is consistently accessible for revisions and for review. As this is a repeat violation, develop a new system for tracking monthly evacuations including the new spot where the written records will be kept and who will be responsible for recording the information and ensuring the record is returned to the assigned spot moving forward. Since this is a program that spends many days outside, your plan shall include considerations made for how these records will be maintained, even when the drills are conducted while children are outside.”

Regulation 3.3.3: The FCCP shall establish a system for taking attendance, including documentation of the time when each child arrives and departs each day he/she attends the FCCH. The FCCP shall save all daily attendance records identifying the hours of children's attendance for at least twelve (12) months from the date that care is provided.

“Upon arrival to the program, there were 8 children in care and no children were signed in on the attendance app. A review of the attendance records reflected that no children were signed in or out of the program on 09/19/2022. The provider confirmed that there were children in care on 09/19/2022. Additionally, while children were consisitently signed into the program for the week of 09/12-09/16/2022, no children were signed out of the program on any of those days. Corrective Action: Immediately plan to sign children into and out of the program in a timely manner. This means that each child's arrival time is recorded within a few minutes of their arrival and each child's departure time is recorded within a few minutes of their departure. Develop a system which will prompt you to record attendance in a timely manner moving forward. The system should be one that will ensure consistent compliance with this regulation. Include details of your new system and who will be responsible for ensuring timely entry of attendance including arrival and departure times moving forward.”

“A review of the evacuation log reflected that evacuation drills were not documented for the months of 09/2018-02/2019. Corrective Action: Create a system which will prompt you to conduct and record evacuation drills every month.”

Regulation 5.2.1.3: The FCCP shall ensure the hand washing procedure include the following steps in order: * Use of warm running water and liquid soap; * Moistening hands with water, applying soap, and washing hands for a minimum of twenty (20) seconds; * Rinsing hands under running water; * Drying hands with a single use towel or paper towel; and then * Disposing of the towel.

“Kathleen stated that during the summer time, while the children are outside all day, she uses a bucket of warm soapy water for the children to wash their hands in before snacks and lunch, rather than having them wash their hands under warm running water.
