Regulation 2.3.9.3: The licensee shall ensure that all staff have met ongoing background check requirements by submitting at least once every five (5) years a Records Check Authorization form to the Division and as required shall submit to fingerprinting.
“The Child Development Division did not receive a new record check authorization form for the Director or Staff A before their 5 year background check renewal date. They did not submit to a fingerprint supported background check prior to their 5 year renewal date. The director was required to renew their fingerprints by 03/01/2023 and Staff A was required to by 05/30/2022. Corrective Action: Immediately send in a new record check authorization forms for the Director and Staff A. Notifiy this licensor when the fingperprint appointments are scheduled. Email a copy of the fingerprint receipts to this licensor. Develop a plan for how the program will track staff fingerprint renewal dates and moving forward will ensure compliance with this regulation.”
