    • Background checks

Regulation 6.2.6.6: The program director shall ensure that no person be left alone with children without approval from the Division based on the results of a background check that includes fingerprinting; with the exception of a parent may be left alone with his/her own child(ren).

“Staff A and Staff B who were substitutes in the Pre-K classroom have not had record check authorization forms sent in to the Child Development Division. Therefore, Staff A and Staff B were working alone with children without completing the fingerprint supported background clearance. It should be noted that there was an emergency in the building that required shifts in classroom coverage. The incident was described as an oversight, the program took immediate steps to come back into compliance. Corrective Action: The program was asked to send in Record Check Authorization forms today, 03/28/2024, for Staff A and Staff B. The program was also asked to replace Staff A or Staff B with another staff member who has completed the fingerprint supported background clearance. As discussed during the visit, please ensure that when substitute coverage is used in the Preschool classrooms a record check authorization form was submitted. If the substitutes will be alone with children they must have completed the fingerprint supported background clearance through the Child Development Division and the Child Development Division confirmed their results were received and they are cleared to work alone with children. Please email this licensor confirming a plan will be put into place to ensure that moving forward complinace with this regulation is met.”

    • Background checks

Regulation 2.3.9.3: The licensee shall ensure that all staff have met ongoing background check requirements by submitting at least once every five (5) years a Records Check Authorization form to the Division and as required shall submit to fingerprinting.

“Seven(7) staff members did not submit to fingerprinting before their five(5) year fingerprint renewal date. Corrective Action: Immediately send in new record check authorization forms in for these staff. As part of corrective action this licensor spoke with the school principal on 5/4/2023 to discuss fingerprint renewal requirements and how to track dates. The program will develop a system to add the fingerprint renewal date to staff files. The program is having a mobile fingerprinting unit come to the school so that staff may complete the fingerprint requirements. Please keep staff fingerprint receipts and notify this licensor with the date that the fingerprints were completed.”

“The children in the room refered to as Jenny's room, were observed to be resting on blankets on the floor. The children in the Mecca room, were resting with blankets on non-porous rest mats. Several of the mats in Mecca room were observed to have large rips which exposed the porous material inside.
