“Between 11/9/2020 and 12/21/2020, CDD sent 2 separate emails to all programs, 1 reminder letter using US Postal Service, 1 reminder letter sent through the program's BFIS account, and a final email reminder. This communication provided the link to be used and CDD contact information for questions or challenges with submitting the report. Licensee [name] stated that this program only runs during the summer and the kids served by this program were likely captured under one of the other MPCC program surveys. She stated that it didn't occur to her that she should enter zeros. Corrective Action: Develop a plan to ensure that an immunzation survey is completed each year during November or December.”
