“The Licensing Specialist observed an Epi-Pen stored at the facility today during the inspection. The child for whom this Epi-Pen is prescribed has an allergy anaphylaxis form on file, but it was completed in 2024. Med Admin forms should be completed again after one year. Please obtain a new med admin or allergy anaphylaxis medication administration form, which is completed by the health practitioner and the parent, and add it to the child's file at the center. Please send a copy to the Licensing Specialist.”

“The center has two Pre-K children enrolled; one of whom is still 4 years old. Licensing Specialist observed electrical sockets in the gym that were uncapped. Programs with children under 5 should have all electrical sockets capped. Please cap these sockets and send photo to Licensing Specialist.”
