“During a review of children's records, it was found that a five year old girl is missing her Health Inventory Part I that should be completed by the parent. Three year old and 1 year 7 month old siblings both need a blood lead test. During the inspection, the Provider texted the parent to inform her about the missing tests. The parent reported that she has an appointment for both of them on 08/14/2024. The appointment will encompass their blood lead tests and she will pick up a copy of their most current immunizations. The immunization records that the Provider currently has shows them as being behind on immunizations, but they are reportedly not behind; only missing the documentation. It was also discovered that a five year old girl is missing another MMR immunization and a one year old girl is missing an MMR and a Varicella immunization according to their ages. Please notify the respective parents of the missing documentation and when received, send a copy of the documents to OCC for the correction of this non-compliance.”

      13A.15.03.04A(1)

“During a review of children's emergency forms, it was found that a five year old girl's emergency form is missing its original date when the parent completed the form and it is missing all of her physician's information. A six year old boy and male siblings, ages 1 year and 7 months old and three years old, are all missing their physician's address on their emergency forms. A one year old girl's emergency form is missing her parent's signature and date. In addition, the same one year old girl does not have any emergency contacts listed on her emergency form. Please notify the respective parents so that they may add the missing information to their child's emergency form, initial and date the emergency form to indicate the updated information. Then send a copy of the emergency cards with the addition of the missing information to OCC for the correction of this non-compliance.”
