Regulation 13.7: Staff and children shall all wash their hands with soap under warm, running water: * before they eat or prepare food * after they use the toilet * after handling animals * after playing outside.
“None of the 17 children present in the program were observed to wash their hands upon arrival at the program or prior eating snack. When asked, management confirmed that the children had come directly from being dismissed from school and did not wash hands. This is a repeat violation first cited 3/15/2021. Corrective Action: Immediately require children to wash their hands with soap and warm, running water at all times required by the rule. Develop a system which will ensure that compliance is maintained for children and staff to hand wash as required, in a manner that it becomes routine.”

Regulation 13.7: Staff and children shall all wash their hands with soap under warm, running water: * before they eat or prepare food * after they use the toilet * after handling animals * after playing outside.
“No children were observed to wash their hands upon arrival to the program or prior eating snack. Further, during free play in the gymnasium space, one child was observed to eat a snack without washing hands. Staff A reported that children generally wash their hands prior to eating snack in the classroom, however there were many distractions on this day which prevented Staff A from providing that oversight. Staff A also stated that children who choose to eat at the gymnasium do not generally wash their hands. Corrective Action: Immediately plan to have children and staff wash their hands with soap and warm, running water at all times required by this regulation. Develop a system which will ensure that all staff have an awareness of this regulation and assume responsibility for ensuring children are washing hands as required, in a manner which will ensure handwashing naturally becomes a part of the routine for the program.”

Regulation 12.14: Cushioning material which is absorbent shall be in place under climbers, slides, swings, or other structures which allow children to achieve a height of more than thirty inches. The depth of surfacing materials shall be appropriate to the height and use of the equipment as outlined by the Consumer Product Safety Commission's National Playground Safety Standards (see Appendix B). Surfacing material shall be maintained in good condition. Grass, bare ground, asphalt and concrete are prohibited under these structures. If weather conditions make the use of equipment unsafe, Staff shall keep children off of the equipment.
“Inspection of the playground revealed that there are several areas that do not meet cushioning material standards as set forth in Apendix B of the regulations. The area where the injury occured, the monkey bars, had 2 inches of compressed mulch and less than an inch of uncompressed mulch. During the vist two other areas of concern were identified. Underneath all 8 swings the mulch is pushed out leaving approximately a foot deep space. Another structure that contains a climbing wall had cushioning material that was so compressed the measurement tool could not penetrate. Vegetation is also growing through the mulch. Some of the growth measured 1-1.5 ft high. Corrective Action: Immediately block access to the swings, monkey bars and climbing wall. Using Appendix B in the regulations and guidance from the Public Playground Safety Handbook ensure that cushioning material is meeting requirements. Email this licensor pictures of the playground when repairs are complete .”
