Regulation 5.4.2.2: Infants shall always be placed on their backs to sleep.
“On 07/29/2026 six (6) month old Child B's mother arrived and found Child A sleeping on their stomach. Child B was not yet rolling over independently. The Director spoke with the two infant staff members and learned that they were placing Child B on their stomach to sleep. This had occured over a period of approximately thirty (30) days. As a result the director reported the incident to the Child Development Division, the lead infant teacher was terminated, the assistant and all staff at the program were required to complete Cribs For Kids, Safe Sleep Ambassador Training. This licensor received the training certificates. Corrective Action: Moving forward ensure all current and any staff hired in the future are placing infants under the age of twelve (12) months on their back to sleep. Status: Appeal Period.”
Regulation 6.1.4.1.4: Furniture and equipment children use for play and daily routines shall be durable and appropriate for the size, abilities, and activities of the children.
“Child A sustained a small bump and bruising to the face after falling from an adult sized toilet, the toilet had a child insert in place, however the insert is designed for toddlers, Child A was fifteen(15) months old at that time. When Child A was thirteen (13) months old they were placed on an open style swing seat instead of the bucket style seat. A staff member reported that she was standing with Child A but after a few minutes Child A let go of the swing and fell onto their left side. No injury was reported. In both instances the parents were notified. Corrective Action: The program will ensure that staff understand the developmental needs of their children and what materials, furniture and equipment is developmentally appropriate for use with the children in their care. Status: Appeal Period.”
Regulation 7.2.2: The licensee shall ensure that all new staff and auxiliary staff, prior to working with children in the CBCCPP; and any new individual designated as the representative of the licensee shall submit a Records Check Authorization form to the Division and shall submit to fingerprinting at a location designated by the Division after having received the Fingerprinting Authorization Certificate by the Division.
“Two (2) staff who have been employed for over sixty (60) days have not yet been fingerprinted. The director said that they both had to cancel multiple appointments due to staffing issues. Corrective Action: By 8/14/2025, email documentation that both staff have completed fingerprints to Licensor [name].”

Regulation 6.2.1.3: The program director shall ensure that the number of staff present meets requirements described in the chart below for ages of children, maximum group size, and staff/child ratios at any time of day.
“On 4/14/2025, the preschool classroom was out of ratio due to a miscalculation in the ages of the children. On that day, there were ten (10) children present with one (1) staff member, including two (2) children who were under thirty-two (32) months old. The program has a variance that allows for a 1:10 ratio if all children are at least thirty-two (32) months old. This is a serious violation. Corrective Action: A link to an age calculator was provided to the director for future reference. By 5/28/2025, please distribute a copy of this report to all enrolled families. The report may be printed and mailed or hand delivered to parents or sent digitally. Additionally, by 5/28/2025, email Licensor [name] a list of all currently enrolled children, including the names and contact phone numbers of their parents or legal guardians. A written statement confirming your understanding of staffing ratio requirements must also be emailed to Licensor [name] by this date.”
