“The Provider only completed one emergency disaster drill during the 2025 calendar year. An emergency disaster drill is required to be conducted and recorded at least twice per calendar year. Please write a letter of correction stating how you will become in compliance with this regulation in the future.”

“During a review of children's records, it was found that two siblings need blood lead tests. One needs his 12 month old blood lead test and the other needs her 24 month old blood lead test. The Provider may correct this portion of this non-compliance by notifying the parent in writing of the need for their children's blood lead test and provide a copy of this written notice to OCC. Those same two siblings also need age-appropriate immunizations along with a 1 year 7 month old boy. Provider was given a written copy of the needed immunizations for these identified children. Please provide their updated immunization records for the correction of this portion of this non-compliance.”

“Provider has not completed the yearly health and safety training. It is entitled 2024 Basic Health and Safety through MSDE. This training was to be completed no later than 12/31/2024. Please complete the training through the MSDE website and forward a copy of the training certificate to OCC for the correction of this non-compliance.”

“During a review of children's records, it was revealed that two children have no evidence of ever having a blood lead test performed. One is one year and 2 months old and the other child is two years old. It was also discovered that two children do not have any evidence of having the appropriate amount of immunizations for their age. Please notify the parents of their child's need for blood lead testing so that they may schedule the testing or so that they may provide results of their child's blood lead test. Also notify the respective parents of their child's need for additional immunizations or have them produce the most current record of immunizations that show the proper amount for their age. For the correction of this non-compliance, please provide a copy of the nofification or blood lead test results and evidence of the proper amount of immunizations.”

“During a review of emergency forms, it was found that three children's emergency forms have not been updated within the last year. Please notify the respective parents and have them review, initial and date their child's emergency form to indicate the update. Please send a copy of the updated forms to OCC for the correction of this non-compliance.”
