The Provider did so on-site when it was brought to her attention. Therefore, this non-compliance was corrected on-site.”

“Seven out of the eight emergency forms have not been reviewed and updated in over a year. Please notify the parents so that they may review their child's emergency forms, update the information, if needed, and initial and date the form to indicate the update. Please send copies of the updated emergency forms to OCC for the correction of this non-compliance.”

“Upon inspection of the Provider's pack-n-plays, it was observed that the Provider has a foam mattress with snugly fitted sheets in both of her pack-n-plays. This is unsafe as the foam mattress, approximately 1.5" thick is a potential suffocation hazard. During the inspection, the Provider removed one of the mattresses, but the other pack-n-play is missing the bottom mat. Therefore, she is only able to use one of the existing pack-n-plays. Currently, the Provider only has one child under the age of one who would use a pack-n-play. During the inspection, the Provider ordered two new pack-n-plays as well as elastic straps to ensure the sheets remain snugly fitting. Please send a picture of the new pack-n-plays without the foam mattress and with snugly fitted sheets for the correction of this non-compliance.”

“During a review of children's records, it was found that a recently enrolled 1 year 4 month old child does not have Health Inventory Part I or Part II in his file. He is also missing his blood lead test results. Provider states that the child's mother was supposed to bring the documents yesterday, but he was not in attendance yesterday. Please forward a copy of these documents to OCC for the correction of this portion of the non-compliance. It was also found that a 6 year old boy needs an additional MMR and an additional Varicella immunization. Please obtain a record of age-appropriate immunizations for him and, while keeping a copy in his file, please submit a copy to OCC for the correction of this portion of the non-compliance.”
