“During the inspection, it was found that the emergency disaster plan had not been reviewed in over two years. When this was brought to the attention of the Provider, she reviewed the plan, initialed and dated it to indicate the yearly review. Therefore, this non-compliance was corrected on-site.”

“During a review of children's records, it was found that a 17 month old girl and a two year old girl have no evidence of having any blood lead testing. Please notify the parents in writing of the need for blood lead testing and provide a copy of that notification to OCC for the correction of that portion of this non-compliance. That is with the understanding that the Provider will obtain the blood lead test results and place them in the respective children's file. It was also found that two children in a sibling group do not have the proper amount of age-appropriate immunizations on file. Please inform the parent of the need for age-appropriate immunizations and the immunization record. Please forward the updated immunization records to OCC for the correction of this portion of the non-compliance.”

“During a review of children's emergency forms, it was found that two siblings are missing all of their physician's information to include the physician's name, address and phone number on each of their respective emergency forms. Please notify their parent so that the parent can add the missing information, initial and date the form to indicate the updated information. Please send a copy of each of their emergency cards with the added information to OCC for the correction of this non-compliance.”

“During a review of children's files, it was found that a 'drop-in' child does not have evidence of having a Health Inventory Part II that is completed and signed by her physician. Please notify the parent so that the parent may obtain this from the physician, place it in her file and send a copy of the Health Inventory Part II to OCC for the correction of this non-compliance.”

“During a review of children's records, it was found that a five year old is in need of another MMR immunization. The Provider stated that she has already notified the parent of her child's need for updated age-appropriate immunizations. Please provide a copy to OCC of the updated immunization record from the doctor when obtained from the parent for the correction of this non-compliance.”
