“The Provider did not complete the 2025 Basic Health and Safety Update until after the due date of 12/31/2025. However, she did complete the course on 01/07/20226 and submitted the certificate of completion to OCC. Therefore, this non-compliance is now corrected.”

“There is no evidence that a boy, who is currently 23 months old, has had a blood lead test. Children must have a blood lead test at 12 and 24 months of age. Please inform the parent of this so that they may obtain the results of previously performed tests or have the testing performed and then submit the results to the Provider. In order to correct this non-compliance, the Provider shall submit the blood lead test to OCC or provide written notification to the parent of the need for blood lead testing and send OCC a copy of that notification.”
