Facility staff #1 had a hire date of 8/29/2024. The disclosure statement on file for facility staff #1 was dated 8/7/2024. The FBI was not completed until 9/14/2024 and the NSOR request was completed 8/30/2024. Facility staff #2 had a hire date of 9/3/2024. The disclosure statement was signed 8/30/202 The FBI was not completed until 9/13/2024 and the NSOR request was completed 9/4/2024. Facility staff #3 had a hire date of 9/15/2024. The disclosure statement was dated 9/6/2024. The NSOR request was completed 9/12/2024. Facility staff #4 had a hire date of 9/3/2024. The disclosure statement was signed 8/27/2024. The NSOR request was completed 9/4/2024. Facility staff #4 required out-of-state clearances and there was not documentation those requests were made at the time of inspection.”

The record showed that facility staff #1 lived in NJ within the past 5 years and there was not out-of-state clearances on file. Facility staff #1 was observed to have their own group of children and during a transition time left a classroom with the group and was not supervised by another qualifying staff person.”
