      Regulation 7.2.2: The licensee shall ensure that all new staff and auxiliary staff, prior to working with children in the CBCCPP; and any new individual designated as the representative of the licensee shall submit a Records Check Authorization form to the Division and shall submit to fingerprinting at a location designated by the Division after having received the Fingerprinting Authorization Certificate by the Division.

      “A staff member submitted their record check authorization form to the Division. The annual assurance letter submitted through the program BFIS account, dated 11/17/25, had informed the provider that the individual must be submit to fingerprinting. The letter provided clear instructions and phone number to call for further assistance. As of 2/19/2026 the individual had not submitted to fingerprinting. The program believed that the fingerprinting completed in October of 2024 was acceptable. Corrective Action: Immediately have the staff submit to fingerprinting. By 3/18/2026, submit a copy of the fingerprinting receipt to this licensor. Ensure the staff member is not left alone with children until after the fingerprint supported background clearance results have returned.”
