“Upon my arrival the provided stated that she was out of compliance with three (3) of the four (4) children present were under the age of two (2). Regulations state that child to staff ratio allows for two (2) children under the age of two (2) with one staff. This is a serious violation. Corrective Action: As discussed during the licensing visit, immediately adhere to ratio requirements. By 5/19/25, submit to this licensor your understanding of child/ staff ratio, ages of children and maximum group size. Complete the parental notification letter as outlined above. By 5/19/2025, submit a list of names and phone numbers of parents who were sent the Parental Notification Letter to this licensor.”
