“Neither hard or electronic copies of completed staff files were not available for review on site. Staff C confirmed that several pieces of required documentation was not available in off site staff files. These items included CPR and first aid certifications and an approved orientation training certificate of completion for Staff A and position qualification and an approved orientation training certificate of completion for Staff B. As a result, compliance with CPR and first aid training and orientation training requirements were not able to be assessed. Corrective Action: Immediately plan to gather required documents to ensure complete staff files for Staff A and Staff B and to maintain completed staff files in either hard or electronic form for all staff on site for review. Develop a system which will prompt the Program Administrator or Program Director to gather all required documentation in a manner which ensures up-to-date staff files for each staff member moving forward. Include details of whom will be responsible for tracking the completion of required trainings and certifications, collecting staff qualification, training and certification documents, how staff will be made aware of the requirements to complete trainings and submit all required documentation in a timely manner, and whom will be responsible for submitting and/or tracking the submission of required documents to Northern Lights at CCV in a manner which ensures compliance moving forward.”
