“A review of staff BFIS credential accounts reflected no evidence that Staff B and C had completed an orientation training. Staff B and C have been with the program between 26 and 42 months and hold position titles which allow them to be left alone with children. An email was sent to the program requesting additional information about these submissions. The response did not include verification that these three staff had evidence of completing a orientation training.
