    • Staff training and qualifications

Regulation 7.1.2.2: Staff who are counted in the staff/child ratios and auxiliary staff left alone with children and/or counted in staff/child ratio as specified in the rule 6.2.1.8 of these regulations shall obtain training in pediatric first aid and in infant and child CPR within three (3) months of beginning work in the CBCCPP and remain currently certified.

“Upon review of staff BFIS Quality and Credential accounts it was identified that the Director's CPR and First Aid certification had expired 11/14/2025. When asked on-site the Director confirmed that they had not become recertified since that expiry. Corrective Action: Following the visit, on 04/08/2026, a renewed infant and child first aid and CPR for the Director was submitted via email. Develop a system to ensure that First Aid and CPR certifications are obtained prior to current expiration moving forward. In the corrective action email provide details of the system developed to ensure compliance moving forward.”

    • Building, equipment and play areas

Regulation 5.10.3.3: Safe Use Zones For all play equipment that is thirty (30) inches or higher from the ground, including climbing equipment, slides, swings and other similar equipment, the licensee shall ensure that the surface under and around the equipment, including recommended use zones, is of approved resilient material that protects children if they fall. * Materials in the use zone shall consist of wood chips, pea stone, mulch, engineered wood fibers, sand, safety-tested shredded or rubber like material or rubber mats designed for protective cushioning; * Materials used in the use zone shall follow the recommendations listed in the most recent publication of U.S. Consumer Product Safety Commission's Public Playground Safety Handbook regarding critical depth of tested materials determined by the height of the highest climbing surface of the equipment; * Materials used in the use zone shall be installed and maintained according to the manufacturer's instructions; * If the loose material such as sand, mulch, or shredded rubber is used in use zones, the licensee shall ensure that the material is raked regularly and replenished to maintain depth and resilience; and * If the resilient material in use zones freezes, staff shall ensure that children will not play on the equipment until the resilient material has thawed and is once again resilient.

“In the fenced outdoor play area, children were observed playing on a large swingset structure to include three (3) swings, a slide, and rock climbing wall. The only cushioning present were rubber mats at the base of the slide, directly beneath the climbing wall, and beneath each swing. The cushioing in place does not meet the requirements as set forth by the U.S. Consumer Product Safety Commission's Public Playground Safety Handbook, as recommended use zones under and around all equipment must contain cushioing material. Corrective Action: Immediately discontinue use of all structures/equipment measuring thirty (30) inches or higher from the ground. By 05/06/2026 ensure that sufficient cushioing material is in place beneath and throughout the safe use zones of all equipment measuring thirty (30) inches or higher. In the corrective action email provide photo evidence that sufficient cushioing material is in place, to include photos of depth measurments and measurements of the cushioned area.”
