Regulation 13.7: Staff and children shall all wash their hands with soap under warm, running water: * before they eat or prepare food * after they use the toilet * after handling animals * after playing outside.

“Hand washing was not observed to occur after playing outside. Other times to observe hand washing did not present at this visit. Corrective Action: Immediately ensure that children and staff wash their hands at times prescribed in this rule. By 12/1/2025, email Licensor [name] a statement that indicates understanding of this rule.”

“One (1) inhaler was not in the original container, and was not labeled in any way. • Building, equipment and play areas

Regulation 11.8: The following shall be maintained in good condition: * roofs * chimneys * interior and exterior walls * doors * skylights * windows * floors * ceilings * stairways * ramps * porches * gates

“In the older children's classroom and stairwell outside the classroom, the wood floors were partially covered. The coverings were damaged and some of the unfinished wood was showing through. In addition, in the older children's classroom, there is an unfinished wooden threshold in the archway between the two (2) rooms that is approximately a foot wide, and some baseboard missing. The licensees said that a flooring contractor was supposed to have come to lay down carpet in the stairwell and finish the floor in the classroom, but the contractor did not show up as scheduled. Corrective Action: Until the contractor fixes or covers the areas, provide undamaged coverings and ensure that the coverings are maintained or replaced if they are damaged and the floor is exposed again. Ensure that the threshold in between the two (2) rooms is treated so that it is non-porous or otherwise easily cleanable. Ensure that the baseboards are replaced and treated so that they are non-porous. In addition, ensure that a vacuum with a HEPA filter is used to clean these areas prior to laying down any coverings, as there may be lead dust accumulated in cracks and crevices in the wood floors. Email photos of both areas to Licensor [name] showing coverings or repair by 12/1/2025.”

    • Records and paperwork

Regulation 4.4: A system for taking attendance, including documentation of the time when each child arrives and departs each day he/she attends the Afterschool Child Care Program, shall be established.

“The program was unable to provide attendance that showed the times each child arrived and departed, only the days they attended. In the Phase III visit conducted on 5/29/2025, Licensor [name] Knight reported that the program intended to utilize a computer application for tracking children's attendance, and that the app will allow for tracking the time of each child's arrival and departure each day. Corrective Action: Moving forward, ensure that attendance that includes the time children arrive and the time they leave is documented daily. Email Licensor [name] a statement of understanding of this rule and a copy of the children's attendance using the app.”

    • Records and paperwork

Regulation 5.28: The Licensee is required to maintain an accurate record of Staff schedules and assignments for all Staff including substitutes.

“The program was unable to provide an accurate daily record of staff schedules and assignments. Corrective Action: Moving forward, staff must sign in and out of classrooms or groups each time they enter or leave, including for break coverage. This may be recorded on paper or digitally. Please email Licensor [name] a statement of understanding of this requirement and specify the method your program will use.”

Correction date in the state record: Sep 25, 2025

    • Supervision and ratios

Regulation 7.1: Each child shall be supervised at all times by Staff as appropriate for the age of the child.

“When departing the building's parking lot, Licensor [name] Knight observed Child X (five years old) alone on the sidewalk near the parking lot without staff supervision. Licensor [name] immediately notified Licensor [name] Rielly, who was driving behind her. As they parked, the child returned inside. Upon approaching the building, Licensors Knight and Rielly observed the exterior door propped open and Child X was standing in the doorway of the interior door, playing with the door. As Licensors Knight and Rielly entered, Child X moved away from the door and hid under a table in the Shul. No staff were monitoring the door at the time and did not see Licensors Knight and Rielly for about a minute after entering the Shul. This is a serious violation. Corrective Action: Due to the seriousness of the violation, a program improvement plan is required, which must include: 1. A plan on how staffing will be arranged in the summer to ensure active supervision is understood, implemented, and monitored; and 2. A plan on how new staff will be onboarded for the summer program. The program administrator must collaborate with SPARQS for support with developing active supervision policies. All current staff must complete at least two (2) hours of training on active supervision by 9/22/2025. All new staff for the summer session must complete the training before the beginning of the session. For support on finding a training that will meet your needs, please contact Northern Lights Resource Advisor Adri Taylo-Behrman at adri.taylo-behrman@ccv.edu. Email the certificates for the active supervision training to Licensor [name] and send them to Northern Lights at CCV to be entered into staff's BFIS quality and credential accounts. Additionally, a hard or digital copy of this report must be provided to all families enrolled at the time of the incident. Email a list of all enrolled children at the time of the incident along with their parents' or guardians' contact details to Licensor [name].”

    • Background checks

Regulation 5.14: The licensee shall ensure that no person shall be left alone with children without approval from the Division which shall be based on the results of the background check to include fingerprinting.

“Upon entering the building, a large group of children were present in the Shul with individuals identified as staff who had not yet submitted record checks to the Division. None of the individuals who have been cleared by the Division to work alone with children were present in the room with the children. In the Phase III visit that occurred on 5/29/2025, Licensing Field Specialist [name] reported that this rule was understood. Additionally, on 5/30/2025, Licensor [name] sent an email to the program detailing this requirement. Corrective Action: Moving forward, ensure that all new staff complete record check authorization forms and send them to the Division on or before their first day of work at the program. In addition, ensure any staff members who have not received their final background clearance from the Division are paired with a cleared staff member who can always see and hear them. Email a plan to Licensor [name] that outlines your understanding of this requirement and what strategies the program will implement to ensure compliance, including how break times and transitions will be managed.”
