    • Supervision and ratios

Regulation 6.2.2.4: The licensed FCCP shall ensure that the number of staff present meets requirements described in the chart below for ages of children, maximum group size, and staff/child ratios at any time of day.

“Upon arrival at the program it was observed that there were only two (2) staff members present with twelve (12) children. The third staff member arrived eleven (11) minutes after the licensors arrived. The program was out of compliance for approximately one hour and twenty-three minutes. When there are twelve (12) children present at the program the regulation requires three (3) staff to be present. This is a serious violation. Corrective Action: Immediately ensure program remains in ratio at all times. Develop a plan to make sure the program is never out of ratio and send it to the licensor by 6/13/2025. Additionally, by 6/13/2025, mail the parental notification letter as outlined above and submit the list of parents, their contact information, and who received the parental notification letter to the licensor.”
