Children should be recorded as being present during each day's attendance. This record should also be verified by the parent. In looking at previous weeks, only some parent signatures were on the attendance form. Please write a letter of correction and send it to OCC stating how you will become in compliance with this regulation in the future for the correction of this non-compliance.”

“During the inspection, it was found that two drawers in the kitchen, which contain knives, have a safety mechanism on them, but it is not working properly. When this was brought to the attention of the Provider, she immediately changed the safety mechanism with a new one for each drawer. They are now properly working. Therefore, this non-compliance was corrected on-site.”

“During a review of children's records, it was found that a one year old, a two year old and a three year old child are missing age-appropriate immunizations. It may be that they have had the immunizations, but the parent hasn't provided the most up-to-date record or it may be that the child still needs the proper immunizations. Please obtain documentation of each of the identified children as having the correct amount of age-appropriate immunizations. Please send a copy of the age-appropriate immunizations for the identified children to OCC for the correction of this non-compliance.”

“During a review of children's records, it was found that a one year old, a two year old and a three year old have no evidence of having any blood lead testing performed. Please inform the respective parents so they may obtain a blood lead test from their child's physician and submit the results of the test to the Provider. “When the Licensing Specialist arrived at the home, the Provider was outside on the playground with two children playing while the Provider was carrying another child on her hip. After talking with this LS for at least 10 minutes, another non-resident child emerged from the home. That child is a child care child who is only two years and seven months old. When asked where that child had been and if she was sleeping, the Provider stated that child went inside the home to use the bathroom. There was no one else inside of the home. This supervision issue was then discussed with the Provider. When this LS was leaving the home, one of the resident children (1 year and 9 months old) and the two year and seven month old child went outside playing on the driveway. A 1 year and 4 month old child was then left asleep in her high chair without any adult with her while the Provider was then trying to get the other two children back inside of the home by verbally redirecting them while remaining on the front porch. Please write a letter of correction stating how you will become in compliance with this regulation in the future.”

“The new play equipment is enclosed by wooden beams to hold the resilient surfacing. These beams are too close to the slide and too close to the swings. Both should have a 6 foot fall zone around each of them. Please remedy this issue and send a picture to OCC showing the correction.”
13A.15.05.04B(3)
“Two drawers that contain hazards in the kitchen did not have properly functioning child proof devices. During the inspection, the Provider placed new child proof locks on both of the drawers now making the hazardous items inaccessible to the children. Therefore, this non-compliance was corrected on-site.”
