“The center has 2 new after school staff persons this year. The notarized release form was received today for both to be cleared. One staff has criminal background checks for the facility but licensing never received the results. Staff persons should not be working until all clearances have been completed. The principal is on-site for the after-school program to supervise the staff.”

“The facility staff must complete the 3-hour basic health and safety training. The facility was reminded about this requirement at previous inspections. The staff must also complete the 2025 basic health and safety update by 12/31/2025. Submit all copies of the completed training certificates to licensing.”

The facility is reminded to notify licensing within the required 5-day timeframe whenever new staff are hired or staff leave employment. “The new pre-k aide has not yet completed the notarize release form. She did her fingerprinting under the archdiocese's authorization number but OCC did not receive the results. The reference number will be utilized to complete a 365-day form. She will complete her notarized release this week and send it to licensing for clearing. New staff may not work with children until cleared by this office. In the future, please ensure all new staff have completed the required clearances prior to beginning work with children. She did complete all clearances required for the school as she is an employee of the school.”
