“The provider shall ensure that a caregiver does not exceed 16 points by using a point system to determine if an additional caregiver is needed. 1. At the start of the inspection, Caregiver #1 was alone and supervising nine children, totaling 26 points. 2. Caregiver #2 left caregiver #1 alone with nine children to run an errand. 3. There were three children between the ages 6 months to 15 months old, two children between the ages of 16 months to 23 months old, and four children between the ages of two years and three years old. 4. The caregiver was over the supervision requirements by 10 points for approximately 30 minutes.”

“Based on review of manufacturer’s instructions and observation, the provider did not ensure equipment used by children was used in accordance with the manufacturer’s instructions. Evidence: 1. The instruction manual for the cribs used by children in states ?Discontinue use of the product when child is able to climb out or reaches the height of 35 inches?. 2. Child #1, three years in age and greater than 35 inches in height was observed napping in a crib. 3. The provider stated three additional children, all with a height greater than 35 inches have been sleeping in the cribs.”
