“When milk was observed in the refrigerator, there was only whole milk present and available. Of the three children that were present during the inspection, one was over the age of two. When asked what milk the Provider gave the three year old, she said that she gives her 2% or whole milk. LS reminded the Provider that children two years old and older are to be served 1% or nonfat milk. Please write a letter of correction stating how you will become in compliance with this regulation in the future.”

“During a review of children's files, a Medication Administration Authorization form for Albuterol was found for two children. However, neither forms were signed or dated by the doctor. In addition, neither form contains the effective dates for the medication to be administered. Please have the parent take the form back to the physician's office so that the physician may complete the form appropriately. Send a copy of the correctly completed form to OCC for the correction of this non-compliance.”

“During a review of children's records, no evidence of a written agreement between the child and the Provider was found for a 9 month old boy, a 3 year old boy or an 8 year old girl. Please submit evidence to OCC of having a contract between each of the identified families for each child and the Provider for the correction of this non-compliance.”

“During a review of children's records, no evidence of any blood lead test was observed in a three year old girl's file. Please provide evidence of this child having had a blood lead test and send a copy of this evidence to OCC for the correction of this non-compliance.”

“Two children, who are enrolled and present in care today, only have an emergency form completed. The Provider does not have any other admission paperwork, including but not limited to, Health Inventory Part I and Part II and evidence of all required immunizations. Please give each set of parents a deadline in which to provide the required paperwork in order to remain in care. Please send a copy of the required documentation for both children to OCC for the correction of this non-compliance.”

“Allegations were made that the television is always on and that the children are allowed to use iPads. During the inspection, the Licensing Specialist observed children using their personal cell phones for games and other internet activities and using tablets. The television was on when they returned inside the home after the bubble activity. After a few minutes, the Provider turned off the television. However, the children, especially the older children were on their phones and iPads the entire time of the inspection, except during the bubble activity and during lunch. For the correction of this non-compliance, write a letter of correction stating how you will become in compliance with this regulation in the future.”
13A.15.08.01F
“During the time that the Provider ended the bubble activity, children were left inside the home alone as well as outside of the home alone, albeit brief. For the correction of this non-compliance, write a letter of correction stating how you will become in compliance with this regulation in the future.”
13A.15.08.01C(2)
“When going back inside the home after the Provider decided to end the bubble activity, she left the two children, ages 6 months and 9 months, in a child infant rocking seat and in the high chair outside and alone while she took the older children inside the home. She then went back outside, leaving the 2 year old child and two 3 year old children along with the older children inside the home. The Provider then picked up the two children, one by one, while still being in their seats and brought them into the home separately. For the correction of this non-compliance, write a letter of correction stating how you will become in compliance with this regulation in the future.”
13A.15.08.01B(4)
“When the Licensing Specialist arrived at the FCC home, children and the Provider were outside and in the backyard participating in a bubble event. The LS observed the Provider in the bubbles with most of the children. However, two children were left approximately 15-20 yards away. They are aged 6 months and 9 months old. They were left strapped in a child infant rocking seat and a high chair, respectively. The Provider was engaged in the bubble activity with the older children. The Provider would be unable to render immediate assistance to one of the children from that distance if one of them required it. For the correction of this non-compliance, write a letter of correction stating how you will become in compliance with this regulation in the future.”
13A.15.04.03B
“Included with those 12 children present, three of those children are under the age of two years old. One child is one year and two months old. Per the Provider's report, as they do not have any emergency forms completed or any other documentation, the other two children are six months old and nine months old. For the correction of this non-compliance, write a letter of correction to OCC stating how you will become in compliance with this regulation in the future.”
13A.15.04.03A
“Upon arrival of the Licensing Specialist, there were 12 children present in the Provider's care and 10 of them were engaged in an outside bubble activity. When asked about being over capacity, the Provider stated that four of them are her relatives. However, the Provider is still providing care for all of the 12 children with none of them being residents of the home. For the correction of this non-compliance, write a letter of correction to OCC stating how you will become in compliance with this regulation in the future.”
13A.15.03.04A(3)
“During a review of emergency forms, it was found that two emergency forms have not been updated in over a year. Emergency forms must be updated, at least, annually. Please give the two emergency forms to the parent (siblings) so that the forms may be updated, as needed, and initialed and dated by the parent. Provide a copy of the two corrected emergency forms to OCC for the correction of this non-compliance.”
13A.15.03.04A(2)
“There are no emergency forms or any other documentation available for five out of the 12 children present in care during today's inspection. Each child in care must have a readily accessible, completed and signed emergency card. Provide a complete emergency form for each child in care to OCC for the correction of this non-compliance and keep them on-file even if they are "drop-in" children.”
