“Based on interview, the center failed to implement the center's procedure to ensure all children are in compliance with immunization provisions. Evidence: 1. The administrator stated the center's policy for immunizations is as follows: Upon enrollment or before the first school day parents need to submit the most up to date immunization records of their child. If somebody does not have an immunization record then we ask them to submit the Religious Exemption Form. 2. The record for child 2 was reviewed. The first date of attendance was documented as 1/31/22. The immunization record was dated 2/25/22. 3. The administrator stated "The parents never gave them the copy when registering. I reminded them a few times but then we were on quarantine for 10 days and got back to school on February 23." She brought it on the 25th.”

“Based on record review and interview, the center failed to have a completed central registry ( CPS ) record check by the end of the 30th day of employment. Evidence: 1. The record for staff 1 documents the hire date as 8/18/21. The copy of the CPS in the file does not have the findings documented. The copy of the check for the CPS is dated 8/17/21. 2. The administrator stated the CPS document was mailed but the findings were never received. They never followed up to find out what happened to the document.”
