“There is one child that the parent is taking a religious objection to the lead testing and immunizations but did not have the doctor sign the form for the lead objection and the parent did not sign the form for Religious Objection to immunizations. The form was left with the provider to have the parent complete. Please send copies of each to the LS once completed.”

“A complaint was received that alleges the children have access to bleach, knives and the provider's insulin bag. The provider said that she had the gate down to the kitchen and a child got a hold of a bottle of bleach, was able to open it, and it spilled on the floor. This occurred when the provider was assisting another child. Since this incident the provider has installed a different type of gate at the kitchen. The gate opens like a door and is self closing self latching. One other occasion the provider had her insulin up on her desk and a child was able to put their hands on it so she moved into the kitchen. She now stores her insulin in the kitchen.”

“One emergency form did not have the parent signature, please have the parent sign and date the emergency form and send a copy to the Licensing Specialist.”

“There is one child that is missing 2 DTAP and 1 Polio immunization. The parent picked this child up while the licensing specialist was conducting the inspection. The issue was discussed with the parent and she stated she thought the child was up to date on all immunizations. She will contact the doctor's office and obtain an updated immunization record or a note from the doctor if she is on a delayed schedule. Another child did not have any Hep B immunizations and the immunization record stated refused. The provider called the parent during the inspection and the licensing specialist spoke to her; she said that she did refuse but thought the child eventually received the immunizations. The parent will look through her records or contact the doctor's office and obtain an updated immunization record. The provider will need to forward these documents to the licensing specialist ASAP to Deborah.shirley@maryland.gov.”
