Regulation 6.2.6.1: Staff shall be able to hear all children of pre-kindergarten age and younger at all times, shall be able to see the children with a quick glance, and shall be able to physically respond immediately.
“On the afternoon of 11/13/2019, a preschool child was left unattended outside for an estimated 10 to 15 minutes. The child was reportedly left outside when his group and staff person transitioned from the gross motor room to the side entrance for the main building. There is unfenced/unblocked river and accessible parking lot within 50 feet of the side door at which the child was found. The ambient temperature at the time was 20 degrees and the child was not wearing a hat or gloves. The child was heard crying by a community member who had pulled into the parking lot between the two buildings. This person talked with the child, identified he was to be in the program, and then brought the child into the front desk area of the Playworks building. Program staff were not aware that the child as missing until he was returned to the group. This is a Serious Violation of supervision requiring a Parental Notification Letter. Corrective Action: As required during the site visit on 11/18/2019, all children are to be supervised at all times in accordance with the rules in Section 6.2.6. This plan will need to include what practices are in place for transitions to and from buildings, transitions of responsibility amongst/between staff persons, and what practices are in place for conducting head counts of children throughout the day. This plan will need to include how staff are trained upon these practices and how management will ensure such practices are followed. Include in the plan the process the Program Director will put in place to identify and address any staffing supervision needs (See Rule 2.2.55). Submit this plan for approval to Licenser Crawford by the noted due date. Regulation 6.2.1.3: The program director shall ensure that the number of staff present meets requirements described in the chart below for ages of children, maximum group size, and staff/child ratios at any time of day.
“Upon review of attendance records between 10/8/2018-12/7/2018 there were six days that more than eight infants; ages 6 weeks to two years were scheduled to attend but resulted in eight or less due to sickness or other absence. Those dates were as follows: 10/16/18 scheduled for nine children, 11/12/18 scheduled for nine children, 11/13/18 scheduled for nine children, 11/26/18 scheduled for ten children, 11/28/18 scheduled for nine children, and 12/3/18 scheduled for ten children. On 10/30/18 there were nine infants in attendance between 9:30-3:00 pm. Three infant room teachers were interviewed who all confirmed that they have eleven children enrolled and on any given day they could have up to ten children in attendance. Staff also confirmed that regardless of the number of children in attendance they function as one group. Management acknowledged that the infant room has operated as one large group and that this practice needs to be adjusted. Failing to maintain group size is a serious violation Corrective Action: Effective immediately, establish and maintain a practice that accomodates max group size being maintained. A program improvement plan detailing the steps to be taken to ensure staff to child ratio and groups size requirements are met at all times shall be submitted for review by 1/21/2019. This plan shall include the staffing plan, anticipated daily attendance, and plans to ensure group size is not exceeded. Mail the Parent Notification Letter as outlined above and submit to this licensor the list of parent's names, their contact information, and the date the parent notification letter was mailed.”
