Regulation 7.1: Each child shall be supervised at all times by Staff as appropriate for the age of the child.
“With staff-communicated permission, a fourth grade child left their outdoor play area and directly entered the program in the gymnasium. The director acknowledged the child as they left the gym and walked down a hall alone to go to the bathroom. The bathroom is located out of sight from the gym. Approximately ten (10) minutes later, this licensor asked the director the whereabouts of the child. The director was uncertain, checked the bathroom and communicated with the child s staff outdoors. The child had exited the school by the door located closest to the bathroom to directly rejoin their group. Corrective Action: Effective immediately, supervise children as required per the rule. Submit a program improvement plan to this licensor by 12/12/24 including the steps that have already been put in place to ensure child safety. The plan must also include how staff will be retrained in supervision practices in children going to and from the bathrooms and the steps taken to ensure supervision of all children is maintained at all times.”

Regulation 12.5: Hazardous substances shall be in their original container, stored separately and inaccessible to children.
“During the visit, two bottles of liquid were located on a small table, near the entrance to the cafeteria. The bottles were within child reach. Staff A stated that the bottles contained a bleach and detergent mixture which was supplied by the school custodian and intended for use on the cafeteria tables. During an activity, Child Z was observed to take one of the bottles and spray it onto a plastic toy. A few moments later, Child Z picked up both bottles, held them with the nozzles pointed away from Child Z and started walking toward other children. When this Licensor made Staff B aware of the situation, Staff B intervened. Staff B placed the bottles back on the table at children's reach. This Licsensor then checked in with Staff A about the issue. At this Licensor's request, Staff A placed the bottles out of children's reach. Corrective Action: Immediately plan to store all hazardous substances in a manner which prevents access by children and to inform all staff of this requirement. Develop a system which establishes a permanant storage place for all hazardous materials, a process for staff to scan each licensed space and remove all hazardous materials from children's reach prior to program operation each day, a process for the proper use and storage of all hazardous materials, and how this requirement will be addressed with all current and newly hired staff mving forward.”
