“Based on a review of the staff handbook and interview, the center failed to establish and implement a procedure to ensure that all areas of the premises accessible to children are free of obvious injury hazards, including providing and maintaining cushioning material under playground equipment. Evidence: 1. A spray bottle of Windex was on the water fountain outside of the children's classroom. The administrator stated the policy for hazardous substances is that they are to be kept out of children's reach. 2. There was no cushioning material under the playground equipment on the older children's playground. The administrator stated there was some under the swings.”

Evidence: The sworn statement for staff 3, start date 3/6/23, documented staff 3 lived in another state within the last five years. The record did not contain documentation of a central registry search, sex offender search, or criminal history name search being completed in that state. Staff 3's employment date was”
