Regulation 6.2.6.1: Staff shall be able to hear all children of pre-kindergarten age and younger at all times, shall be able to see the children with a quick glance, and shall be able to physically respond immediately.
“On 9/3/2025 a child left the playground towards the end of rest time on their own. The child was located in less than five (5) minutes on the other side of the building and was sitting on a large rock. The program director immediately notified the family of the incident and created a new system for staff and children. Orange cones have been placed along the side of the building for a better visual cue and construction vests for children were ordered for children to wear. The gate of the small playground will be kept closed when they are only using that space. They have reduced staff to child ratio from 1:10 to 1:8 for the preschool group when they are utilizing the outdoor play space as children are waking up from rest time. This is a serious violation. Corrective Action: Continue with the corrective action that was implemented directly after this incident of lack of supervision. By 4/1/2026, mail or email the parent notification letter as outlined above. Submit the list of parent's names, contact information and when they received the parent notification letter to this licensor.”
