Regulation 5.4.2.3: The FCCP and staff shall ensure that infants sleep in cribs or port-a-cribs. Infants shall not be placed to sleep on soft surfaces, such as a couch or sofa, pillow, quilts, sheepskins or blankets. Infants shall not be placed in car seats, swings, high chairs, jumping chairs or similar equipment for sleep. A child that falls asleep in this type of equipment shall be gently transferred to a crib or port-a-crib as soon as feasible.
“Upon this licensor's arrival, a child under one (1) year of age was sleeping in their car seat. Attendance records indicated that they had been in care since 7:40AM and the time of the visit was 11:00AM. The provider indicated that the child does sleep in the car seat regularly at the program. Corrective Action: Effective immediately, ensure infant safe sleep procedures are followed at the program and all children in care utilize a cot, mat, port-a-crib or crib for sleep based on what is developmentally appropriate and safe for their age. A statement of compliance shall be sent to this licensor describing the sleep procedures for children under one (1) year of age going forward.”

Therefore, the extinguisher had not had an annual inspection in three (3) years. Corrective Action: It is required to get the extinguisher inspected and tagged. It is also required to develop a system to manage the inspection due date so that inspections are not missed going forward.”
