    • Health, medication and illness

Regulation 5.1.2: General Health Examinations: Within forty-five (45) days of enrollment, the licensee shall obtain documentation of the child's age appropriate well care exam from the parent. Documentation shall include information regarding any health conditions and medications that may impact the care of the child.

“In a review of 3 child files, none of them had well visit documentation. Corrective Action: Ensure that all children have documentation of a well visit in their files. Create a plan to ensure that this document is collected for each child within 45 days of enrollment. Notify sarah.crosier@vermont.gov when all enrolled children have this document in their files, and send the plan to this licensor for review.”

    • Supervision and ratios

Regulation 6.2.6.1: Staff shall be able to hear all children of pre-kindergarten age and younger at all times, shall be able to see the children with a quick glance, and shall be able to physically respond immediately.

“A child exited the program building, crossed the sidewalk, and entered the road in front of the building. The road is Route 7, a numbered highway. The child was found by a motorist and returned to the program by the motorist. The child was able to exit through the front door of the building and the gate outside the front door to get to the road. Corrective Action: Ensure a system is created to restrict children's access to leaving the building without staff knowledge. This could include alarms, bells, sturdy gates and/or additional locks. Please consult with Division of Fire Safety if additional locks are the chosen system. This is a serious violation which requires that a Parental Notification Letter be sent to all families of children enrolled in your program. Please also send to sarah.crosier@vermont.gov names and contact information for parents who will be sent the Parental Notification Letter.”

Infants shall not be placed to sleep on soft surfaces, such as a couch or sofa, pillow, quilts, sheepskins or blankets. Infants shall not be placed in car seats, swings, high chairs, jumping chairs or similar equipment for sleep. A child that falls asleep in this type of equipment shall be gently transferred to a crib or port-a-crib as soon as feasible.

“Staff stated that a child had been left sleeping in a swing for approximately one hour on one or more occasions. Corrective Action: Develop a plan to ensure that no infant is left sleeping in any equipment other than a crib or pack and play, and that they are moved as soon as possible if they fall asleep in other equipment. • Staff training and qualifications

Regulation 6.2.4.3: The program director shall ensure that each group of children is led by a staff member who is at least qualified as a teacher associate for at least 75% of the hours of operation.

“Two classrooms do not currently have lead teachers who meet requirements for teacher associate or higher. Staff A has 9 related credits based on information in BFIS and provided to CDD. Staff B has 3 related credits. This program does have an expedited variance, and would require at least 12 related credits to meet teacher associate qualifications.
